Personalized Care Where You Live

In-home hospice care allows patients to remain close to family, pets, personal belongings, and familiar routines. Care may be provided in:

  • Private homes and apartments
  • Board-and-care homes
  • Assisted living communities
  • Memory care communities
  • Other qualifying residential settings

Our interdisciplinary team works with the patient, family, attending physician, and residential caregivers to develop an individualized care plan. Services are adjusted as the patient’s condition and needs change.

What Does In-Home Hospice Care Include?

Skilled Nursing and Physician Support

Nurses visit according to the care plan to assess the patient, monitor symptoms, provide appropriate care, and educate family caregivers. Hospice physicians and medical directors oversee clinical services and coordinate with the attending physician.

Pain and Symptom Management

We create a personalized approach to managing pain, shortness of breath, nausea, weakness, anxiety, restlessness, and other symptoms. Our goal is to help patients remain as comfortable as possible.

Personal Care Assistance

Certified home health aides provide respectful assistance with bathing, dressing, grooming, hygiene, mobility, and other activities of daily living included in the care plan.

Medications, Supplies, and Equipment

Serenity Hospice Care coordinates medications, supplies, and medical equipment related to the qualifying hospice diagnosis. Depending on individual needs, this may include a hospital bed, oxygen equipment, wheelchair, walker, or personal care supplies.

Coverage depends on hospice eligibility, the patient’s insurance plan, and whether the service or item relates to the hospice diagnosis.

Emotional and Spiritual Support

Social workers and counselors help patients and families navigate emotional concerns, practical challenges, and community resources. Non-denominational spiritual support is available in accordance with personal beliefs and preferences.

Support Available 24/7

Patients and caregivers can contact our on-call hospice team 24 hours a day, seven days a week when questions arise, or symptoms change. We evaluate each situation and coordinate additional assistance when appropriate.

Benefits of Hospice Care at Home

In-home hospice care offers:

  • Comfort in familiar surroundings
  • Personalized routines and schedules
  • More opportunities for family time
  • Less stress from traveling for care
  • Caregiver education and guidance
  • Proactive symptom management
  • Coordinated interdisciplinary support

Familiar surroundings may be especially reassuring for people living with Alzheimer’s disease or another form of dementia.

Who May Qualify?

In-home hospice may be appropriate for people with a life-limiting illness who choose comfort-focused support rather than curative treatment. Eligibility requires a clinical assessment and physician certification.

We support eligible patients with advanced cancer, heart failure, lung disease, dementia, end-stage renal disease, neurological conditions, and other qualifying illnesses.
