Hospice Referrals for Healthcare Professionals

Your Partner in Coordinated Hospice and Comfort Care

Serenity Hospice Care collaborates with physicians, hospital discharge planners, case managers, social workers, and care coordinators throughout Southern California. Together, we help eligible patients transition to personalized hospice care in their homes or residential communities.

Based in Claremont, California, we support patients across Los Angeles, San Bernardino, Orange, and Riverside counties.

Coordinated Support for Your Patients

Timely hospice referrals can give patients and families more time to benefit from symptom management, caregiver education, emotional counseling, and practical assistance.

Our interdisciplinary team communicates with referring providers, attending physicians, patients, relatives, and facility staff. This coordinated approach supports continuity of care while respecting each patient’s clinical needs, personal preferences, and comfort goals.

We provide hospice services in:

  • Private homes and apartments
  • Board-and-care homes
  • Assisted living communities
  • Memory care communities
  • Skilled nursing facilities

Why Refer to Serenity Hospice Care?

Accredited and Certified Care

Serenity Hospice Care is Joint Commission-accredited and Medicare-certified. Our team follows established standards for clinical quality, patient safety, documentation, and coordinated care.

Responsive Referral Support

Our intake team is available 24 hours a day, seven days a week to discuss potential referrals, explain eligibility requirements, and coordinate an assessment.

Interdisciplinary Clinical Services

Depending on the individualized care plan, patients may receive support from hospice physicians, nurses, certified home health aides, social workers, spiritual counselors, bereavement professionals, and volunteers.

Pain and Symptom Management

Our clinical team develops individualized strategies for pain, dyspnea, nausea, fatigue, anxiety, restlessness, and other distressing symptoms associated with the qualifying illness.

Attending Physician Collaboration

Patients may continue working with their attending physician. Our hospice medical director and nurses coordinate the plan of care and provide relevant clinical updates in accordance with applicable requirements and patient preferences.

Coverage Verification

Admissions coordinators help verify benefits through Medicare, Medi-Cal, Medicare Advantage, and private insurance. Coverage depends on eligibility, the individual plan, and whether services relate to the hospice diagnosis.

Clinical Indicators for a Hospice Evaluation

A hospice evaluation may be appropriate when a patient experiences:

  • Recurrent hospitalizations or emergency room visits
  • Progressive functional or mobility decline
  • Increased dependence with activities of daily living
  • Unintentional weight loss or reduced nutritional intake
  • Recurrent infections or impaired skin integrity
  • Increasing pain, dyspnea, fatigue, or other symptoms
  • Advanced cardiac, pulmonary, renal, neurological, or oncological disease
  • Progressive Alzheimer’s disease or another form of dementia
  • A shift from curative treatment toward comfort-focused care

These indicators do not independently establish eligibility. Hospice admission requires a clinical assessment and physician certification in accordance with the applicable benefit requirements.
