About Serenity Hospice Care
Compassionate Hospice Care in Claremont and Southern California
At Serenity Hospice Care, we believe every person deserves to experience life’s final chapter with comfort, dignity, and respect. Our mission is to enhance each patient’s quality of life while providing families with compassionate guidance, practical assistance, and peace of mind.
Based in Claremont, California, we provide hospice and comfort care throughout Los Angeles, San Bernardino, Orange, and Riverside counties. Patients receive individualized support in private homes, board-and-care homes, assisted living communities, and skilled nursing facilities.
By managing pain and other challenging symptoms, we help patients remain as comfortable as possible. This allows families to focus on what matters most: sharing meaningful time with the people they love.
Our Mission and Philosophy
Our hospice philosophy centers on the needs, choices, beliefs, and wishes of every patient. We care for the whole person – not only their medical condition. That means addressing physical discomfort as well as emotional, social, psychological, and spiritual concerns.
Our goal is to:
- 24/7 on-call hospice support
- Pain and symptom management
- Care at home and in residential settings
- Emotional, social, and spiritual guidance
- Support for patients, families, and caregivers
- Joint Commission-accredited hospice provider
A Coordinated Team Dedicated to Your Family
Serenity Hospice Care is a Medicare-certified and Joint Commission-accredited hospice agency. Our interdisciplinary team collaborates with the patient, family, primary physician, and residential care staff to develop and oversee a personalized hospice care plan.
Depending on the patient’s needs, the team may include:
Hospice physicians and medical directors:
Coordinate medical services and collaborate with the patient’s attending physician.
Registered nurses and licensed vocational nurses:
Assess changing needs, manage pain and symptoms, provide clinical care, and educate family caregivers.
Certified home health aides:
Assist with personal hygiene, mobility, and activities of daily living.
Medical social workers:
Offer counseling and connect families with practical, financial, and community resources.
Spiritual and bereavement counselors:
Provide emotional and non-denominational spiritual support that respects individual beliefs.
Trained volunteers:
Offer companionship, friendly visits, and additional support for patients and families.
The Values Guiding Our Care
Compassion First
Every patient is treated with empathy, patience, and respect. Care is tailored to the individual – not simply to a diagnosis.
Clinical Excellence
Our professionals provide attentive medical supervision, pain management, symptom control, and ongoing assessments for people living with advanced illnesses.
Respect for Personal Choice
Patients and families receive the information they need to participate in care decisions. We honor individual goals, preferences, and values whenever possible.
Support for the Entire Family
Hospice affects more than the patient. We provide education, counseling, caregiver guidance, and 24/7 on-call support to help families feel informed and supported.
